中文摘要:
目的:探讨跟骨钢板内固定治疗跟骨关节内骨折的临床应用效果。
方法:采用钢板内固定治疗跟骨关节内骨折13例,男10例,女3例;年龄26～50岁,平均38岁。按Sanders分型Ⅱ型3例,Ⅲ型9例,Ⅳ型1例。对临床结果进行分析、总结。
结果:术后随访时间4～22个月,平均16个月。术中无重要神经、肌腱损伤,术后无切口感染和深部感染,1例于骨折愈合后取内固定时发生切口小范围坏死;无骨不愈合和内固定松动及再骨折发生;1例于久走后出现距下关节痛,尚能忍受。按张铁良百分评分法进行评定优6例,良6例,可1例,总优良率为92%。
结论:跟骨钢板内固定治疗跟骨关节内骨折具有操作简单、有效恢复B hler角和Gissane角、固定坚强、利于早期踝关节功能锻炼的优点,是目前治疗跟骨关节内骨折较为理想的方法之一。 |

| Operative treatment of intra-articular calcaneal fractures with calcaneal plate |
|
ABSTRACT
Objective: To explore clinical effect of intra-articular fractures of calcaneus with calcaneal plate*$.
Methods:Thirteen cases of intra-articular fractures of calcaneus were classified by the standard of Sanders,including 3 cases of type Ⅱ,9 cases of type Ⅲ and 1 case of type Ⅳ.Among them,10 cases were male and 3 cases female;the patients were 26 to 50 years old with average 38 years of age.All the patients were treated with internal fixation of calcaneal plates (type “Y” and “T”).
Results:All the patients were followed up from 4 to 22 months with an average of 16 months.No injury of muscle tendon and nerve,infection of incision,bone delayed union,plate and screw loosening and refracture were found during and after the operation in all of patients except one who complained pain of talocalcaneal joint after long distance walking.According to Zhangtieliangs standard with one hundred as a full score,the excellent,good and fair curative effects were achieved in 6,6 and 1 cases respectively with an excellent and good curative rate of 92%.
Conclusion:Open reposition and internal fixation with calcaneal plate (type “Y” and “T”) is an ideal method in treatment of intra-articular fractures of calcaneus.The advantages of this method includes:①simple procedur;②satisfactory recovery of Bhler angle and Gissane angle;③firm fixation;④earlier exercise of ankle joint. |
