目的：探讨关节镜下抽出钢丝固定治疗儿童胫骨髁间棘骨折的临床疗效.
方法：自2008年10月至2010年12月,手术治疗儿童胫骨髁间棘骨折21例,男14例,女7例;年龄7～14岁,平均10岁;手术时间为伤后3～8d,平均4d.术前常规摄X线片,行CT扫描或MRI检查了解骨折情况及关节损伤情况;手术全部采用关节镜下抽出钢丝固定治疗;术后早期功能锻炼,摄X线片了解术后情况,并采用Lysholm膝关节评分进行临床症状和功能评分.
结果：所有患者骨折愈合,Lysholm膝关节评分由术前的30.2±4.2提高至术后的92.8±9.1;优18例,良3例.
结论：关节镜下抽出钢丝治疗儿童胫骨髁间棘骨折的优点是经济,固定牢固,属于稳定性固定,且钢丝直径小而光滑,对骨骺医源性损伤较小. |

| Arthroscopic treatment of tibial intercondylar eminence fracture in children |

Abstract:
Objective：To study clinical effects of extractable steel wire for the treatment of tibial inter condylar eminence fracture in children under arthroscopy.
Methods：From October 2008 to December 2010,21 children with intercondyloid eminence fractures were treated with fixation by extractable steel wire under arthroscopy.Among the patients,14 patients were male and 7 patients were female,ranging in age from 7 to 14 years old with a mean of 10 years old.The period from injury to operation ranged from 3 to 8 days,with an a verage of 4 days.Before operation,X-ray,CT and MRI were used to determine fracture and joint injury type.Lysholm criteria were used to evaluate clinical effects.
Results：All fractures were healed.The Lysholm score improved from preoperative 30.2±4.2 to postoperative 92.8±9.1;18 patients got an excellent result and 3 good.
Conclusion：The extractable steel wire in treatment of tibial intercondyloid eminence fractures in children under arthroscopy has advantage of economics,firm fixation,in favour of early functional exercise,decreasing the opportunity of bony avulsion of the anterior cruciate ligament and less iatrogenic in jury of osteoepiphysis due to small diameter and slick steel wire. |
