创伤性浮膝的治疗

随着交通建筑业的快速发展,高速高能量损伤明显增多。自2000年以来,收治创伤性浮膝28例,施行手术固定治疗,效果满意。1临床资料本组28例,男21例,女7例;年龄18～62岁,平均42岁。致伤原因:交通伤21例,坠落伤5例,重物压砸伤2例。按王亦璁教授分型^[1]:骨干骨折15例,双髁部骨折6例,骨干合并同侧髁部骨折7例。其中胫腓骨开放骨折5例,包括GustiloⅠ型1例,Ⅱ型3例,Ⅲ型1例。合并颅脑损伤1例,胸部损伤2例,同侧髌骨骨折2例,创伤性休克5例。2治疗方法首先处理危及生命的合并伤,同时对伤肢暂行外固定以稳定骨折端。待生命体征相对平稳后行骨钉牵引,夹板固定以制动。
