分期手术治疗Ⅲ型Pilon骨折24例

自1999年10月-2004年6月采用的分阶段治疗Ⅲ型Pilon骨折24例效果满意,现将体会报告如下。1临床资料24例Pilon骨折患者中男15例,女9例;年龄17~65岁,平均36岁。致伤原因:高处坠落伤20例,车祸4例;均属胫骨远端粉碎骨折压缩型（Ruedi-AllgowerⅢ型^[1]）,均为闭合骨折。2治疗方法伤后急诊将单侧多功能外固定支架置于小腿外侧,近侧2枚螺钉选用密质骨螺钉,置于胫骨骨折近端,远端2枚选用松质骨螺钉,置于跟骨上。安装支架后用延长器适当延长,至关节间隙及轴线正常。腓骨骨折可采用钢板固定,切口位于腓骨的侧后方,尽量远离日后将采用的踝前内侧正中切口位置。
