肱骨髁上管狭窄症2例报告

病例1:男,12岁,因右肘上内侧疼痛伴右手麻木无力1年为主诉入院。入院查体:右肘内髁上4cm处可触及一骨性隆起,挤压可诱发前臂掌侧及手掌部麻木、疼痛。大鱼际肌萎缩,拇外展对掌功能减弱,握拳无力,手掌桡侧三指半及背侧三指半远侧皮肤感觉减退。初步诊断为肱骨髁上管狭窄症。于臂丛阻滞麻醉下行髁上突切除,正中神经探查松解术。术中见由髁上突及Struther韧带形成的髁上管狭窄压迫正中神经,切除髁上突及Struther韧带,解除对神经的压迫,术后神经受压症状消失。随访半年,神经功能完全恢复正常(见图1)。
