老年股骨粗隆间粉碎骨折未固定小粗隆26例疗效观察

由于股骨粗隆间粉碎性骨折非手术治疗存在着很高的病死率（住院病死率达10.5%）及髋内翻发生率（46%）^[1],自2002年1月-2005年1月应用动力髋螺钉（DHS）治疗老年股骨粗隆间粉碎骨折26例,均未行小粗隆骨折块固定,术后至少随访半年以上,取得良好的疗效。1临床资料本组26例中,男11例,女15例;年龄65~87岁,平均77.8岁,均为顺粗隆间骨折。左侧10例,右侧16例。跌伤24例,车祸伤2例。按Evans标准分为ⅢA型19例,Ⅳ型7例。BMD显示均伴有不同程度的骨质疏松,合并糖尿病3例,高血压冠心病4例,慢性支气管炎肺气肿肺心病7例,脑梗死2例。2治疗方法采用切开复位行DHS内固定。
