手法治疗肱骨外科颈骨折合并肩关节脱位

自2001年6月-2005年6月采用手法复位治疗肱骨外科颈骨折合并肩关节脱位45例,疗效满意,现介绍如下。1临床资料本组45例中男28例,女17例;年龄35～55岁,平均为42岁。交通事故伤14例,高处坠落伤10例,梯坎滑跌伤12例,平地跌伤9例。脱位类型:盂下型10例,喙突下型33例,锁骨下型2例。均合并肱骨外科颈骨折,受伤距复位时间1h～2d,平均7.2h。2治疗方法患者取仰卧位,伤肩作臂丛麻醉,并肌注曲马多100mg,外展伤肢,两助手分别固定骨折的远近端,顺势作适度的拔伸牵引,矫正骨断端上移,使肱骨干端与肩盂拉开一定的间隙,然后在无牵引的条件下,稳定于外展45°位置。
