目的:探讨可吸收螺钉治疗踝关节骨折的临床疗效。
方法:应用可吸收螺钉治疗踝关节骨折58例,男31例,女27例;平均年龄28岁。三踝骨折5例,内踝骨折27例,外踝骨折26例,其中内外踝骨折9例,合并下胫腓联合分离6例,合并腓骨下段骨折4例。单纯内、外踝骨折采用2枚可吸收螺钉固定;同时内外踝骨折各骨折分别用2枚可吸收螺钉固定;合并下胫腓联合分离者,从腓骨向胫骨拧入1枚可吸收拉力螺钉固定胫腓关节;内踝合并腓骨下段骨折者,内踝用1或2枚可吸收螺钉,腓骨骨折用重建钢板内固定;三踝骨折者据骨折片大小分别每一骨折用1或3枚螺钉内固定。
结果:50例均获得随访,时间1~4年,平均26个月,经Kennedy踝关节功能标准评分,优30例,良20例。无骨折延迟愈合及骨折不愈合、固定钉松动、踝穴不稳定、畸形愈合,无明显创伤性关节炎等严重并发症。
结论:可吸收螺钉是种新型内固定材料,内固定治疗踝关节骨折疗效好,可免除二次手术,值得推广。 |

Biodegradable screws for the treatment of fractures of ankle joint |
|
Abstract:
Objective:To explore the therapeutic effects of biodegradable screws for the treatment of fractures of ankle joint.
Methods:From 2000 to 2004,fifty-eight patients with fractures of ankle joint(male 31,female 27,mean age 28 years)were treated with biodegradable screws.Among them,5 patients were trimalleolar fractures,27 were fractures of medial malleolus,and 26 were fractures of lateral malleolus,in which 9 patients were bimalleolar fractures,6 patients were accompanied with separation of distal tibiofibular junctures,4 were accompanied with distal fractures of fibula.Simple fractures of medial malleoluse or lateral malleolus were treated by fixation with two biodegradable screws;compound bimalleolar fractures were treated by fixation with two biodegradable screws in each fracture.For fractures combined with separation of distal tibiofibular junctures,one biodegradable screw was implanted from fibula to tibia to fix tibiofibular joint.For fractures of medial malleolus combined with distal fractures of fibula,one or two biodegradable screws were used to fix medial malleolus and reconstruction plate was used to fix fibula.For trimalleolar fractures,one or three biodegradable screws were implanted into each fracture.
Results:Fifty patients were followed up from 1 to 4 years,with an average of 26 months.According to Kennedy’s evaluation criteria of ankle function,30 patients reached excellent results and 20 good.There were no complications such as delayed union or non-union of fractures,loose of screw,unstable of ankle joint,malunion and obvious traumatic arthritis.
Conclusion:As a new internal fixation method,the biodegradable screw is effective to reach excellent results in treating fractures of ankle joint without second operation.This material is worth of spreading. |
