高龄股骨颈基底部粉碎性骨折人工股骨头置换术

随着人们生活水平的提高,高龄人群增多,发生股骨颈及粗隆间骨折呈逐年上升趋势。由于高龄股骨颈基底部粉碎性骨折,常伴有严重骨质疏松、骨折块复杂,处理十分棘手。近3年我科处理16例此类病例,报告如下。1临床资料16例中女13例,男3例;年龄76～101岁,平均84.5岁。骨折粉碎块:3块9例,4块5例,5块2例。合并同侧Colles骨折3例,合并对侧Colles骨折1例。合并糖尿病7例(血糖11.1～23.0mmol/L),高血压8例,脑血管病4例,冠心病6例。2治疗方法16例均在硬膜外麻醉下行髋关节前外侧入路,选用骨水泥型双极人工股骨头,戴围领14例,不戴围领2例。
