股骨颈骨折术后钢钉滑入臀后误诊1例

股骨颈骨折手法复位,经皮钢针内固定治疗已广泛应用于临床,但钢针滑入骨盆外误诊在盆腔内未见报道,现将我院收治的1例报告如下。患者,女,68岁,右股骨颈外伤性骨折术后1年于2001年10月13日入院。1年前骨折经手法复位,电透下经皮3枚鳞纹针内固定,术后穿“丁”字鞋1周出院;后因感右髋关节酸胀不适,要求取内固定再次入院,患者大小便正常。查体:行走自如,无跛行,双髋关节活动正常,臀部未扪及包块,足背动脉搏动良好,患肢感觉、运动无异常。X线片可见130mm钢针游离在盆腔,骨折已愈合。治疗

方法:在连续硬膜外麻醉下,仰卧位取腹部直肌旁切口,探查膀胱、子宫等盆腔脏器正常,暴露髋臼内缘亦未发现金属异物。
