上举牵引整复法治疗急性肩关节前脱位

1999年7月-2003年6月,作者采用外展上举牵引法整复急性肩关节前脱位35例均获成功,现结合文献对这一整复方法以及决定急性肩关节前脱位预后的相关因素作一探讨。1临床资料本组35例,其中男28例,女7例;平均年龄39岁（13~71岁）。所有患者均有外伤史,多为初次脱位。急诊检查:患肩功能丧失,方肩畸形,原关节盂空虚,Dugas征阳性。标准肩关节正位片和肩胛骨侧位（轴位片）诊断肩关节前脱位,其中合并大结节撕脱性骨折5例。2治疗方法患者仰卧位,术者位于患侧,1%利多卡因20ml关节腔内局部麻醉后,患肢上臂中立位,肘关节屈曲自然搭于术者前臂,术者一手握患肢上臂远端持续牵引,徐徐外展达90°时。
