腕关节色素绒毛结节性滑膜炎1例

患者,女,34岁,4年前右手大鱼际处在提重物牵拉损伤后出现疼痛肿胀,理疗热敷后症状消失,期间疼痛肿胀反复发作,1周前再次发作时右腕掌侧发现一质软、轻度触痛的肿物,前来就诊。专科检查示:右腕尺掌侧可触及1个直径约4.0 cm的肿块,质软、界限不清,皮温正常无红肿,轻度触压痛。右手大鱼际处见明显肿胀,皮色正常,皮温稍高,触压痛阳性,右手拇指活动尚可,末梢血循良好,拇、食指掌侧感觉略减退。X线检查未见骨质异常,术前诊断为腱鞘巨细胞瘤。于臂丛麻醉下行肿物切除术,右上臂驱止血带,右腕纵行掌偏尺侧切口,切开皮肤、皮下、深筋膜,即见色黄肿物,沿腱鞘走行,形状不规则并向远端延伸。
