手法治疗陈旧性第4、5掌腕关节脱位1例

患者,男,31岁,于2004年4月13日与人斗殴时右拳不慎击到墙壁上,引起右手背疼痛、肿胀,明显畸形,由于工作繁忙和误认为是软组织损伤,患者自行在家口服及外搽药物治疗,经治疗17 d后,疼痛及肿胀减轻,但手背隆凸及握拳功能障碍未消失,遂来我院就诊。检查见右掌背尺侧第4、5掌骨基底部隆凸畸形,掌腕关节活动障碍,右手握拳功能障碍,压痛较轻,无尺神经损伤体征。X线检查示右手第4、5掌腕关节脱位,第4、5掌骨基底部向尺侧及背侧移位,与钩状骨重叠。诊断为右手第4、5掌腕关节陈旧性脱位。检查示脱位的第4、5掌骨基底部无活动度,1人复位未成功,后用治疗关节僵硬的中药外洗验方外洗40min。
