极度外展法整复肩关节前脱位

我院采用传统复位方法治疗肩关节脱位失败16例,结合受伤姿势,采用极度外展牵引复位成功,报告如下。1临床资料本组16例,男15例,女1例;年龄15~43岁。右肩11例,左肩5例,均为直接暴力损伤,其中训练伤12例,坠落伤2例,牵拉伤2例。伤时上肢均处于上举位置。16例均为前脱位:盂下型4例,喙突下型8例,锁骨下型4例;合并有大结节撕脱者4例。有3例患者复位前患侧上肢有麻木感,复位后恢复,无血管、神经损伤。2治疗方法复位时间伤后20min~10h。体弱、肌肉薄弱者直接复位,肌肉强壮及伤后时间长肌肉肿胀紧张性高者肌注度冷丁75~100mg。
