胸腰椎骨折内固定术后椎弓根钉折断的原因分析

椎弓根是椎体后方最坚强的结构,通过它可获得脊柱三柱固定。经椎弓根螺钉内固定治疗的技术在我国已应用了十几年,它的固定效果可靠,但也出现了一些并发症,其中断钉及螺钉松动的问题更是不容忽视。当内置物松动、折断后,已恢复高度的受伤椎体将再次发生塌陷、生理前凸丢失,严重者又转变成后凸畸形。本文就18例胸腰椎骨折术后发生断钉的病例进行分析,以探讨螺钉断裂的原因。1临床资料2000年3月-2004年12月,共收治胸腰椎骨折患者243例,其中行椎弓根钉内固定者192例。按照Denis分型,压缩型83例,爆裂型104例,骨折脱位型5例。
