盘外置管法注射胶原酶溶解髓核的实验研究

目的:观察盘外置管法注射胶原酶是否有利于髓核的溶解以及胶原酶的注射剂量与滴速对髓核溶解的影响。

方法:实验中模仿临床的盘外置管法,将手术中摘取的髓核在体外与胶原酶反应,并且改变给药的速度和剂量,运用免疫组化的单克隆抗体技术对髓核中Ⅰ、Ⅱ、Ⅲ型胶原进行原位定性、定量研究以及HE染色对髓核中胶原的形态进行观察。

结果:缓慢滴注胶原酶可使髓核中胶原含量明显减少,尤其是Ⅰ、Ⅱ型胶原的溶解更充分。

结论:采用盘外置管法慢速滴注胶原酶,可提高胶原酶注射的精确性,延长胶原酶与髓核的作用时间。

An experimental study on nucleus pulposus dissolution via guttatim epidural injection of collagenase
Abstract:

Objective:To investigate whether or not the guttatim epidural injection with collagenase is helpful to the dissolution of nucleus pulposus and the effects of the dosage and rate of guttatim epidural injection on the dissolution.

Methods:In this in vitro study,the surgically-removed human disc material was incubated with the enzyme.The rate of injection and the doses of collagenase were changed during the experiment.Monoclone antibodies of types Ⅰ,Ⅱ and Ⅲ collagens and HE staining technique were used.The types Ⅰ,Ⅱ and Ⅲ collagens in situ were quantitatively detected and the discolysis was investigated.

Results:More nucleus collagens,especially types Ⅰ and Ⅱ collagens,were digested by guttatim epidural injection of collagenase at low speed.

Conclusion:Guttatim epidural injection at low speed can enhance the accuracy of the injection and extend the reactive time of collagenase with the nucleus collagens.Thus,guttatim epidural injection at low speed is beneficial to treat the lumbar disc herniation.
