脊柱后路显微内窥镜术与硬脊膜及神经根损伤的初步探讨

脊柱后路显微内窥镜（MED）术是目前治疗腰椎间盘突出症较理想的手术方式[1-3],如何提高MED术的安全性及治疗效果,一直以来是颇受关注的问题。由于本术式在1.5 cm内径的通道内操作,术野小而深,操作不慎或术野不清极易造成硬脊膜及神经根损伤,从而影响手术疗效。我院自2001年3月引进美国枢法模公司生产的MED系统,至2003年10月共施术287例,针对工作初期遇到的问题,我们对如何预防及处理MED术时硬脊膜及神经根损伤的问题,进行了初步探讨。1资料与方法1.1临床资料本组287例,男153例,女134例;年龄21~76岁,平均43.5岁;病史1个月~30年,平均38个月。
