多方向牵引治疗移位的跟骨关节内骨折

跟骨关节内移位骨折是临床上较棘手的问题,我院自1995年至2003年采用多克氏针多方向牵引治疗跟骨关节内骨折56例,疗效满意,现总结如下。1临床资料本组56例,男42例,女14例;年龄16~62岁,平均36岁。骨折分型,本组均为距下关节内骨折,根据Essex Lo-前X线片均显示患足足弓有不同程度的塌陷,跟骨短缩变宽,B hler角变小或消失,术前测B hler角0°~15°,平均4°。2治疗方法根据骨折粉碎骨块数量及其移位方向,确定穿针的数量、位置及牵引方向,一般每一骨块穿1枚克氏针。在局部麻醉、C形臂定位下,行骨牵引,牵引总重量为3~8 kg,牵引方向根据骨块移位情况调整。
