腰椎结核病灶清除脓肿内大出血

腰椎结核手术合并血管损伤大出血以前有过个案报导,但发生在腰大肌脓肿的静脉大出血尚未见报导。我院自1978-2000年在行腰椎结核腰大肌脓肿病灶清除脓肿搔爬术时,发生6例腰大肌脓肿内静脉大出血,现将其出血情况、处理方法及脓肿腔内的3例静脉变异情况介绍如下,以供同道参考。1临床资料6例大出血患者,均为女性,年龄22~46岁,其中L2,3结核1例,L3,4结核1例,L4,5结核4例,均有腰大肌脓肿。手术入路:右腰背入路1例,其余5例均是右侧倒“八”字腹膜外入路。出血量1400~7000ml,死亡1例。
