局部注射类固醇类药物致跟腱断裂2例

例1,男,56岁,入院前5d在下台阶时,自感“嘭”的一声,即感觉右跟部疼痛,行走不稳。查体所见:皮肤无破损,跟骨结节上2cm处触及空虚感,跟腱连续性中断,压痛（+）,Thompson试验（+）,提踵试验（+）,X线片示骨质无异常改变,诊断为“跟腱断裂”。既往于4个月前患者右跟部疼痛,在当地医院诊断为“跟腱滑囊炎”,行氢化泼尼松注射液封闭治疗,7~10d1次,连续注射7次。术中所见:切开皮肤、皮下,见跟腱在附丽点上2cm处完全断裂,断端呈马尾状,颜色黄暗,少许肉芽组织,清理断端间组织,将跟腱断端修剪平整（切除部分送病理）,将近端用钢丝倒“U”形穿绕,自足底皮肤拉出,结扎在纽扣上,细丝线间断缝合跟
