关节镜术加益气补肾行血汤治疗膝骨性关节炎疗效观察

目的:探讨关节镜手术加内服益气补肾行血汤治疗膝骨性关节炎(OA)的临床疗效和对关节液、血清中IL-1、IL-6水平的影响。

方法:将96例OA患者随机分成治疗组(47例)和对照组(49例)。治疗组采用关节镜手术配合内服益气补肾行血汤4个疗程(7d为1个疗程),对照组单纯采用关节镜手术,然后观察术后1个月的临床疗效和随访1年的复发情况,并检测术前和术后1个月血清与关节液中的IL-1I、L-6的含量。

结果:治疗组疗效优于对照组(P<0.05),两组关节液、血清中IL-1、IL-6含量水平均明显下降(P<0.01),且治疗组下降幅度优于对照组(P<0.05)。随访1年治疗组复发率7.9%,对照组复发率30.3%。

结论:中药内服配合关节镜手术治疗OA近期疗效满意,能明显改善膝关节功能。

Outcome of arthroscopic operation plus administration of qi-invigorating,kidney-supplementing,blood-activating decoction (益气补肾行血汤) on treatment of knee osteoarthritis
Abstract:

Objective: To explore the outcome of arthroscopic operation plus oral administration of qi-invigorating,kidney-supplementing,blood-activating decoction on knee osteoarthritis and influences on the level of IL-1,IL-6 in serum and synovial fluid.

Methods:Ninety-six osteoarthritis patients were randomly divided into two groups:a treatment group(47 cases)treated by arthroscopic operation plus oral administration of qi-invigorating,kidney-supplementing,blood-activating decoction for 4 weeks and a control group(49 cases) treated by simple arthroscopic operation.Clinical outcome were observed for 1 month after operation,and the relapse was observed during a period of 1-year follow up.The level of IL-1,IL-6 in serum and synovial fluid were detected respectively before operation and 1 month after operation.

Results:The clinical outcome of the treatment group was superior to that of the control group(P<0.05).The level of IL-1,IL-6 in serum and synovial fluid was greatly declined in both groups (P<0.01),and the descent range of the treatment group was superior to the control group (P<0.05).The follow-up for one year showed that the relapse rate was 7.9% in the tratment group and 30.3% in the control group.

Conclusion:Knee osteoarthritis treated with arthroscopic operation plus oral administration of qi-invigorating,kidney-supplementing,blood-activating decoction can achieve satisfactory short-time effect.
