难复位性肩关节脱位的手法治疗

我科自2001-2004年对7例难复性肩关节脱位采用改良足蹬法复位,取得满意效果,介绍如下。1临床资料本组7例,男5例,女2例;年龄27~65岁,平均44岁。均为右侧,且都是喙突下脱位。最少整复过3次,最多11次。病程1~8d。4例伴大结节撕裂骨折,2例为习惯性脱位,1例腋下伴广泛皮下出血(多次多种手法所为),2例为臂丛麻醉下复位失败者。就诊时均摄肩关节X线片,患肩呈方肩畸形,弹性固定,Dugas征阳性,直尺试验阳性。2治疗方法使用改良手法时均不需麻醉。复位时,手法大致同手牵足蹬法,只需将水平牵引改为外展、前屈(伸)并后旋30°~60°。
