髋臼骨折脱位合并坐骨神经损伤的治疗

自1990年1月-2002年10月,共收治髋关节骨折脱位合并坐骨神经损伤患者38例,获得满意效果。1临床资料本组38例,男26例,女12例;年龄18～62岁,平均40岁。左侧24例,右侧14例;按Letournel分类:前柱骨折5例,前壁骨折4例,后壁骨折14例,后柱骨折9例,横形骨折3例,双柱骨折3例。合并颅脑外伤3例,胫腓骨骨折4例,腹部多发损伤3例。2治疗方法2.1保守治疗5例髋臼骨折程度较轻,后脱位合并坐骨神经损伤,均行闭合牵引复位和牵引治疗4～6周。2.2手术治疗33例在受伤后7d内采用后侧入路切开复位内固定并行坐骨神经松解术。
