髋关节脱位合并股骨头骨折的治疗

自1995-2003年收治合并髋关节脱位的股骨头骨折22例,现就治疗的有关问题加以讨论。1资料与方法1.1一般资料本组22例中男16例,女6例;年龄20～64岁,平均35岁。左髋8例,右髋14例。交通伤17例,高处坠落伤5例。髋关节后脱位18例,前脱位4例。合并膝关节韧带损伤4例,髌骨骨折2例,股骨骨折2例,胫腓骨骨折1例;颅脑外伤3例,胸、腹部损伤各1例。采用Pipkin创伤病理分型[1]:Ⅰ型6例合并髋关节脱位,股骨头骨折位于中央凹的远侧;Ⅱ型8例合并髋关节脱位,股骨头骨折位于中央凹的近侧;Ⅲ型3例合并髋关节脱位,股骨头骨折伴股骨颈骨折;Ⅳ型5例股骨头骨折伴有髋臼骨折。
