吻合小隐静脉的腓肠神经营养血管皮瓣在跟踝区皮肤缺损的应用

足跟踝部皮肤缺损较常见,且局部软组织较少,治疗比较困难。自2000年1月-2003年12月,采用吻合小隐静脉的腓肠神经营养血管皮瓣修复跟踝区皮肤缺损13例,效果良好,现报告如下。1临床资料本组13例,男9例,女4例;年龄5～48岁,平均26岁。损伤原因:交通伤9例,挤压伤2例,慢性溃疡(均为石膏压迫所致)2例。皮肤缺损面积:4cm×5cm～5cm×12cm。13例均有肌腱及骨外露,急症手术6例,亚急症手术7例。2手术方法以外踝与跟腱之间中点与窝中点连线即腓肠神经体表投影为皮瓣轴心线,以5、7、9cm处为腓肠神经营养血管的穿出点,以踝上5～6cm为轴点,依据创面大小形状设计皮瓣。
