胸腰椎骨折前路治疗进展

脊柱骨折的治疗,一直是脊柱外科关注的课题,随着胸腰椎前路固定器械的发展与生物力学研究的深入,前路治疗方法和观点也有了进一步的发展。本文对国外近年来胸腰椎骨折前路治疗方面的进展进行综述。1手术指征的确定胸腰椎爆裂性骨折骨块向后压迫神经伴神经症状时,是前路手术的主要适应证。但要依骨折类型、损伤平面和神经损伤程度,再确定行前路或前后路联合手术。无神经压迫症状时,应考虑脊柱后凸畸形的程度,因为畸形>15°时可引起晚发性腰背痛,>30°时可发生晚发性神经损伤[1]。在L2平面以下应避免单独行前路融合术,因腰段生理性前凸,不利于植骨愈合,术后假关节发生率较高,特别是L4平面,前路器械的放置会增加大血管损伤。
