手法整复胸骨体骨折1例

患者,男,55岁。从3m高车顶部坠下,前胸着地,当即呼吸困难,不能直立。查体:患者身体向前屈曲,不能伸背直立,双手护前胸,呼吸困难,前胸正中胸骨部压痛明显,局部肿胀,胸骨部畸形。胸骨体侧位X线片显示:胸骨体骨折,远端向前移位,断端上下重叠10cm(见图1)。手法整复:局麻下,嘱患者仰卧于硬板检查床,取头低足高位,背后垫薄枕,两手上举过头,使两肩后伸,胸部向前挺。嘱患者吸气张胸,使胸骨体向前过伸成角,术者双拇指紧压于骨折断端处,配合呼气时向后挤压,即可听到“咯嗒”一声响,表明复位成功。
