带锁髓内钉固定配合中药外敷治疗中重度胫腓骨开放骨折

1999年4月-2002年12月,采用带锁髓内钉固定配合中药外敷治疗中重度胫腓骨开放性骨折43例,报告如下。1　临床资料本组男34例,女9例;年龄18～50岁,平均29岁。受伤至手术时间3～10h,平均6h。内固定材料:扩髓型髓内钉7例,非扩髓型髓内钉36例。按Gustilo分型:Ⅱ型26例,ⅢA型13例,ⅢB型3例,ⅢC型1例。均为单下肢骨折,胫骨的骨折部位均适合带锁髓内钉固定要求。合并肋骨骨折5例,脑震荡3例,全身多处软组织挫伤29例。2　治疗方法早期彻底的清创,清除坏死及污染严重的组织。选择合适长度及直径的髓内钉,顺向打入髓内钉,直视下复位;利用瞄准器,将远端及近端的锁钉锁上。
