跟距关节内跟骨骨折移位变化的CT测量

目的:测量跟距关节内跟骨骨折移位的具体数据,分析其移位规律。

方法:对66例跟骨骨折患者双侧跟骨同时行冠状面CT扫描,以健侧跟骨CT冠状面上能完整显示后距下关节面、载距突和跟骨体的一幅扫描面图像为基准,分别对健侧和患侧跟骨进行测量,包括冠状面上跟骨最远端的宽度、跟骨中部的宽度、跟骨长度及Perie角,并进行统计学分析。

结果:患侧前跟部宽度较健侧增宽966mm(P<0.01);冠状面中央部跟骨平均增宽881mm(P<0.01)。Perie角患侧3215°,健侧1740°,两者相差1475°(P<0.01)。跟骨的长度患侧为5567mm,健侧为6059mm,差异无显著意义(P>0.05)。

结论:跟距关节内跟骨骨折以最远端横向移位最大,故此在治疗上应高度注意这种病理变化,彻底矫正其横向移位。

CT measurement of fragments displacement of intraarticular calcaneal fractures
Abstract:

Objective:To measure the fragment displacement of intraarticular calcaneal fractures and to analyze pattern of the displacement.

Methods:66 patients with intraarticular calcaneal fractures were scanned with coronal CT for both calcaneus at same time.Taking uninjured side of the calcaneus as standard,which showed clear images of the articular surface of the subtalar joint,sustentaculum of talus and the calcaneal corpus,the width of front part and middle part,length and Perie’s angle were measured and compare for the both calcaneus.

Results:The average width in the front parts and in the middle parts of the injured calcaneus respectively increased 9.66 mm and 8.81 mm compared with the uninjured side (P<0.01).The average Perie’s angle was 32.15° at injured side and 17.40° at uninjured side (P<0.01).The average length of calcaneus respectively was 55.67 mm in injured side and 60.59 mm in normal side (P>0.05).

Conclusion:The horizontal displacement is the most remarkable at the front part of intraarticular calcaneal fracture.Such pathological changes should be paid attention to in the treatment.
