膝骨性关节炎分级治疗的疗效分析 |

目的:评价非手术治疗、关节镜和全膝关节置换术治疗不同严重程度的膝骨性关节炎的效果。
方法:对采用这三种方法治疗的213例(304膝)患者,依据膝骨性关节炎严重性指数(ISOA)及Kellgren-Lawrance(K-L)X线分级标准不同进行分组,对治疗前后的ISOA进行回顾性分析。
结果:经3~62个月(平均20.6个月)的随访,K-L分级Ⅰ、Ⅱ和Ⅲ级且ISOA<14者,非手术治疗组和关节镜组治疗前后ISOA均有显著性差异;K-L分级Ⅲ级且ISOA≥14、Ⅳ级者,全膝关节置换术组治疗前后ISOA有显著性差异。
结论:K-L分级Ⅰ、Ⅱ和Ⅲ级且ISOA<14者,应选择非手术治疗或关节镜治疗;K-L分级Ⅲ级且ISOA≥14、Ⅳ级者,全膝关节置换术是唯一有效的方法。 |

The treatment of different severity of the gon osteoarthritis:an analysis of the therapeutic effects |

Abstract:
Objective:To evaluate the effect of non operative methods,arthroscopy and total knee arthroplasty in the treatment of different severity of the gon osteoarthritis.
Methods:According to the index of severity for osteoarthritis of the knee(ISOA) and Kellgren Lawrance(K-L)grades of X ray diagnosis,213 cases(304 knees),treated by the three methods were divided into different groups,the scores of ISOA before and after treatment were analyzed.
Results:During a 3 to 62 months(20.6 months in average) following up,there were significant difference of the ISOA before and after treatment in the non operative methods and arthroscopy groups for the cases with K-L grade Ⅰ、Ⅱ and Ⅲ,ISOA<14,and in the total knee arthroplasty group for the cases with K-L grand Ⅲ,ISOA≥14 and K-L grand Ⅳ.
Conclusion:The cases with K-L grade Ⅰ、Ⅱ and Ⅲ,ISOA<14 which should be treated by non operative methods or arthroscopy.For the cases with K-L grand Ⅲ,ISOA≥14 and K-L grade Ⅳ,the total knee arthroplasty is the only way. |
