胸腰椎爆裂骨折机制及保守治疗的研究进展

胸腰椎爆裂骨折治疗方法的选择在临床上常常依据损伤的严重程度而定,包括椎管阻塞率、伤椎后突畸形角度以及有无脊髓神经损伤等。由于该类骨折存在机械不稳定与潜在的神经不稳定,主张手术的作者认为早期进行减压、固定有利于脊柱与神经的稳定与恢复。而提倡保守治疗的作者认为,对于无脊髓神经损伤的胸腰椎爆裂骨折,虽然有潜在的不稳定因素,但经过保守治疗而取得良好的远期疗效却是令人满意的。近20年来,由于影像学和生物力学的发展,对胸腰椎爆裂骨折的损伤机制、病理变化以及稳定性等方面有了不少新认识,这无疑给保守治疗提供了新的理论依据。
