股骨巨大缺损双向钢板内固定1例

患者,男,31岁,因右下肢外伤肿胀、畸形、疼痛、功能严重障碍8d,于1999年11月10日下午转来我院。患者8d前乘坐摩托车与对面开来的大卡车相撞,患者坐于后位只觉右下肢剧痛,即刻昏迷40min左右,及时送县医院救治后苏醒。查体:右大腿中下1/3肿胀、畸形,扪及散乱的骨擦感,闻及骨擦音,右大腿下段前方有伤口长5cm,已缝合4针。功能活动严重障碍,但伤肢踝关节及足趾活动感觉存在,能扪及足背动脉搏动。X线片示:右股骨中下1/3骨缺损长约16cm,并残存多块细小碎骨片(块),右股骨髁间多块骨折阴影。入院诊断:①右股骨中下1/3开放性巨大缺损合并髁间粉碎性骨折;②右小腿软组织损伤;③头伤。
