扩大窦道病灶清除治疗脊柱结核回顾分析

目的:分析扩大窦道病灶清除术治疗脊柱结核的远期疗效及术后后凸畸形的变化情况。

方法:856例脊柱结核,经扩大窦道切除病灶清除治疗,其中一次手术同时清除两侧病灶768例,二次以上者88例,术中同期植骨436例。

结果:经平均89.7个月的随访,参见方先之制定标准,治愈756例,好转75例,优良率为97.1%,436例植骨者的融合率为96.3%,后凸角度为4°～57°,平均153°。

结论:扩大窦道切除病灶清除治疗脊柱结核疗效满意,同期行植骨融合对预防后凸畸形稳定脊柱效果可靠。

Retrospective analysis on treatment of vertebral tuberculosis by enlarging sinus and clearing focus

Abstract:

Objective:To analyze the effect of vertebral tuberculosis treated by enlarging sinus and clearing focus,and the changes of post operative kyphosis retrospectively.

Methods:856 cases of spine tuberculosis were treated by the way of enlarging sinus and clearing focus(clearing two sides focus in one operation) 768 cases,more than twice operations 88 cases,bone grafting 436 cases.

Results:In the follow up of the mean period 89.7 months,according to Fang’s standard,756 cases were cured and 75 cases were improved.97.1% of the patients had good results,96.3% had solid body fusion,and the kyphosis was 4 to 57 degrees(average 15.3 degrees).

Conclusion:The enlarging sinus and clearing focus in the treatment of spinal tuberculosis have satisfied effect,fusing with bone graft meanwhile was effective for correcting kyphosis and stabilizing vertebral column.
