腋下型肱骨头脱位伴臂丛受压急诊治疗5例

腋下型肱骨头脱位在临床上较为少见,作者在临床中遇到5例报告如下。1临床资料5例患者中有4例女性,1例男性,年龄最大72岁,最小55岁,平均65.2岁。受伤机制均为上肢处于外展位时沿上臂纵向暴力。5例就诊时表现姿势特殊,受伤的上肢处于外展上举位并弹性固定,不能内收,伴有剧烈的疼痛并需他人扶持伤肢维持于外展位(图1),4例患者伴有不同程度的肱骨大结节撕脱骨折。5例患者均有不同程度臂丛神经受压症状,表现为环指小指麻木,或上臂、前臂感觉过敏,1例桡动脉搏动减弱,患手紫绀,末梢循环不良。
