折顶复位Colles骨折致骨片嵌压正中神经1例

患者,男,22岁,战士,因摔伤致左腕肿痛畸形1h就诊,查体:一般可,左腕肿胀、压痛、餐叉样畸形,活动受限,左手诸指感觉、运动、血运可,X线片示左桡骨远端骨折。遂在牵引下给予屈腕尺偏折顶手法复位、小夹板外固定,复位后患者自觉左手掌桡侧半及拇、食、中指麻木,稍松解夹板后亦不减轻,查体:一般可,餐叉样畸形纠正,在左腕背伸位时于近侧腕横纹中点处可触及一与皮肤表面垂直的骨片尖端,压之向左手拇、食、中指掌面有放射性麻痛,Tinnel征阳性,左手诸指运动、血运可。
