18例骨折后脂肪栓塞综合征的诊治

脂肪栓塞综合征(FES)是创伤骨科的严重并发症之一。其发病急骤,病势凶险。一旦发生,若不及时诊断和处理,将会导致严重肺、脑损伤,重者因多脏器功能衰竭而死亡。现报告我院创伤骨科及ICU曾收治的18例FES,并结合文献复习,对其早期诊断和治疗进行讨论。1临床资料18例中男13例,女5例;年龄32～46岁(平均38.7岁)。所有病人栓塞前无胸、脑外伤等引起的肺实质及脑实质病变。血SaO₂、PaO₂、肾功能、电解质及心电图均正常。其中股骨干骨折4例,股骨干骨折合并胫腓骨骨折8例,单纯胫腓骨骨折2例,股骨干骨折合并脊柱骨折3例,踝关节骨折1例。
