人工髋关节置换术输异体全血致急性肝损害一例

患者女性,62岁,体重52kg,因左股骨颈骨折行人工股骨头置换术,既往有支气管扩张病史30年,肺气肿,无肝炎病史。入院检查:血型“O”型,WBC5.8×10⁹/L,RBC3.66×10¹²/L,HGB103g/L,HCT0.331,PLT19l×10⁹/L,GPT10.0IU/L,GOT16.0IU/L,TP83.1g/L,GLT46.0g/L,A/G0.80,GLU6.40mmol/L,乙肝五项均阴性,丙肝抗体阴性。入手术室常规监测BP130/75mmHg（1mmHg=0.133kPa）,HR90次/分,SPO₂97％,T37.0℃。于L_3,4穿刺行腰麻硬膜外联合麻醉,阻滞平面T₁₁-S₅。术中因血压下降静点麻黄素10mg2次;氟芬合剂1ml（氟派啶1.25mg加芬太尼0.025mg）,立止血1ku,艾司洛尔60mg,地塞米松10mg。手术95min,术中出血约500ml,尿量170ml,补充乳酸钠林格氏液1000ml,血定安500ml,补异体“O”型全血200ml（已保存1周）。
