儿童移位性伸直型肱骨髁上骨折治疗方法的选择

肱骨髁上骨折是儿童肘部最常见的骨折。最常用的治疗方法是闭合复位,石膏托外固定,牵引,闭合复位经皮克氏针内固定和切开复位内固定。移位骨折的治疗问题很多,包括神经、血管损伤,获得和维持复位困难,以及肘关节僵硬或肘内翻畸形。本文就1990年1月至1996年9月93例移位性伸直型肱骨髁上骨折不同方法治疗的结果进行报道。1 临床资料1.1 一般资料　男74例,女19例;年龄从16个月至14岁;左侧60例,右侧33例,大多数发生在6～10个月(58例,62.4％)伤后就诊时间从1小时至20天。按照简单的Gart-land系统[1]进行分类。Ⅱ型38例,Ⅲ型55例。1.2 伴随损伤　①骨折:同侧前臂损伤4例,尺桡骨骨折2例,桡骨下段骨折1例,Colles骨折1例;对侧桡骨小头骨折1例。1例尺桡骨骨折和1例Colles骨折行闭合复位石膏托外固定,其余3例切开复位内固定。②神经损伤:正中神经损伤3例,其中有1例合并肱动脉损伤;桡神经损伤1例,Volk-mann缺血挛缩1例。
