孟氏骨折伴桡神经骨间背侧支损伤的治疗

目的:对孟氏骨折合并桡神经骨间背侧支损伤的原因,临床表现,治疗结果进行分析。

方法:自1990年～2000年5月共诊治了6例患者,均为儿童。新鲜骨折手法复位石膏外固定3例,切开复位尺骨钢板内固定2例,均未做神经探查;陈旧性骨折尺骨畸形矫正+神经探查1例。

结果:新鲜骨折合并神经损伤5例均自行恢复,陈旧性骨折畸形矫正神经探查者术后半年恢复肌力3级。

结论:孟氏骨折合并桡神经骨间背侧支损伤大多数为神经牵拉伤经观察保守治疗能够恢复,但应当注意少见的桡神经嵌顿在肱桡关节时必须早诊断和早手术探查。

Treatment of posterior interossous nerve injury caused by Monteggia fracture

Abstract:

Objective:6 cases of Monteggia fracture combined with posterior interossous nerve injury were reviewed

Methods:6 cases of Monteggia fracture with posterior interosseous nerve injury were treated since 1990～May to 2000.Of them,5 cases were fresh fractures and treated by manual reduction or open reduction,without exposure of the nerve.1 case was an old fracture with ulna healed up in deformity and treated by osteotomy of the ulna with plate fixation,the nerve was exposed and neurolysis was done.

Results:The nerve injury in 5 fresh fractures recovered in 3 months,the patient who was treated with neurolysis was follwed up for 6 months;a grade Ⅲ of the muscular strength was recovered.

Conclusion:Most of the nerve injuries caused by Monteggia fracture could recovered with conservative treatment;the radial nerve entrapment is a rare condition,however it should not be neglected.
