髋臼骨折手术治疗的探讨

目的:探讨髋臼骨折的治疗方法。

方法:26例髋臼骨折按Letournel分型,后壁骨折12例,后柱伴后壁骨折4例,横行加后壁骨折3例,T型骨折5例,前柱加后半横行骨折2例。根据不同骨折类型,分别采用加压螺纹钉、重建钢板或螺钉钢丝内固定等方法,术后进行良好的功能锻炼。

结果:26例病人平均随访17个月,根据赵炬才等提出的疗效评定标准:优18例,良4例,尚可3例,差1例。

结论:正确的诊断分型对手术入路及内固定的选择起着决定性作用,有利于关节的重建,确保良好的功能恢复。

Operative treatment of fracture of acetabulum

Abstract:

Objective: To investigate the surgical treatment of fracture of acetabulum

Methods: 26 cases of fracture of acetabulum were classified according to Letournel standard,in which 12 cases had posterior wall fracture,4 fractures of the posterior column with posterior wall,3 transverse fractures with posterior wall,5 T type and 2 anterior with posterior hemitransverse fracture.The patients were treated with compression screw,reconstructive plate or screw and wires according to the different types of the fracture.

Results:26 cases were followed up with an average period of 17 months.According to Zhao’s curative effect evaluation criterion,the excellent results were obtained in 18 cases,good in 4,fair in 3 and poor in 1.

Conclusion:The accurate diagnosis and classification is the key for the selection of surgical procedure and internal fixation,is beneficial for joint reconstruction and to ensure functional recovery.
