儿童股骨颈骨折的治疗

目的：探讨儿童股骨颈骨折的治疗方法与预后的关系,减少儿童股骨颈骨折的远期并发症。

方法：26例患儿先行短暂牵引复位后行三枚交叉光滑细克氏针闭合穿针内固定,其中经骨骺型2例、经颈型12例、经基底型11例、粗隆型1例,术后早期不负重功能锻炼。

结果：术后24例经1到7年随访分析,全部骨性愈合,据毛宾尧儿童股骨颈骨折疗效判定标准,22例功能良好,无股骨头无菌性坏死发生。

结论：三枚交叉细克氏针闭合穿针内固定、早期积极功能锻炼、晚期负重是治疗儿童股骨颈骨折的有效方法。

Treatment of femoral neck fractures in children

Abstract:

Objective:To explore the relation between the method of treatment and prognosis and to reduce the late complication of femoral neck fractures in children.

Methods:26 cases of femoral neck fractures in children were internally fixed by 3 thin Kirschner pins,including Delbet Colonna type Ⅰ 2 cases,Ⅱ 12 cases,Ⅲ 11 cases,Ⅳ 1 case.All had exercise in bed 2～3 weeks after surgery.

Results:24 cases were followed up from 1 to 7 years and all had solid bony union.According to the criteria of MAO Bin yao’s,22 cases had excellent hip function and no necrosis of femoral head occurred.

Conclusion:3 thin Kirschner pins internal fixation,early exerce and weight earing in the late period is effective methods of treatment for femoral neck fractures in children.
