目的:通过研究三枚空心拉力螺钉经皮治疗股骨颈骨折临床操作方法,手术前后注意事项,达到提高临床疗效,减少股骨颈骨折的并发症。
方法:本组41例均为新鲜股骨颈骨折,经术前牵引复位,在牵引状态及C型臂机透视下,从患肢大腿外侧穿刺,与股骨成20°~30°角的方向交叉钻入导引针,定位正确后,再分别拧入3枚空心拉力螺钉。
结果:随访2年2个月~13年2个月,采用北京积水潭医院疗效评定标准,41例中愈合39例,愈合率95.12%,发生股骨头缺血性坏死6例,占14.63%.
结论:术前良好的复位,术中选择合适的螺钉,正规的操作,是手术成功的关键,骨折愈合的基础。术后早期不负重,正确的康复锻炼是降低股骨头缺血性坏死发生率的必备条件。 |

Treatment of femoral neck fracture with percutaneous three cannulated screws fixation |

Abstract:
Objective:To improve the curative effect and reduce the complication of femoral neck fracture through studying the clinical performance of percutaneous three cannulated screws fixation
Methods: After reduction by preoperative traction,with the aid of C arm X ray device,41 cases of fresh femoral neck fracture were fixed with 3 cannulated lag screws which were inserted laterally along its guide pin at an angle of 20°~30° degrees with the shaft of the femur.
Results:After follow up of 2 years and 2 months to 13 years and 2 months,39 cases healed(the rate of healing were 95.12%) and necrosis of femoral head occurred in 6 cases according to the criteria of Beijing Jishuitan Hospital.
Conclusion:The excellent preoperative reduction,suitable screw fixation,and standard manipulation were the key points of success;of operation while the postoperative rehabilitation and rational medicine administration were the prerequisites of reducing the incidence of avascular necrosis of the femoral head. |
