目的:研究颈椎斜扳法下寰齿关节的安全性。
方法:运用生物力学中的电测技术,设计了一种斜扳模拟装置,采用7具新鲜尸体颈椎标本,对寰齿关节在不同生理病理状态,拔伸与非拔伸条件下的接触应力进行观察。
结果:斜扳时局部接触应力在拔伸状态、生理状态和病理失稳状态下之比为1∶1.2∶9.4.正常生理状态下斜扳,后伸位应力大于前屈位而小于自然中立位。
结论:颈椎斜扳时患者头部以微屈位为最好,同时施加一定拔伸力。对颈椎存在严重退变失稳的患者,应减小旋转的幅度和力量。 |

Abstract:
Objective: To study the safety over the atlanto odontoid joint of the c ervical spine during oblique pulling man ipulation.
Methods: The contact stress of the atlanto odonto id joint in physiologic unstable conditi ons with and without pulling and extendi ng of seven cadaveric specimans of cervi cal spine were observed with a biomechan ical electric measuring apparatus and ob lique pulling device.
Results:The ratio of contact stress in pulling and extending conditi on,physiologic condition and pathologic condition is 1∶1 2∶9 4.Under normal phys iologic condition,the stress in posterio r extension is greater than that in ante rior flexion,but less than in neutral po sition.Conclus ion A better option of the manipulation posi tion is to put the patient′s head at a s light flexion when a certain pulling ext ending force is applied.In patient with severe degenerative changes of the cervi cal spine and unstability,the amplitude of rotation and the force of manipulatio n should be decreased. |
