高龄股骨颈骨折的治疗

目的:探讨高龄股骨颈骨折的治疗方法。

方法:采用手术并配合应用中医中药治疗移位性股骨颈骨折。手术方式为闭合复位X线电视监视下经皮或小切口多根螺纹针固定36例,三角针固定6例,加压螺丝钉与螺纹针协同固定(子母钉)25例,人工股骨头置换10例。

结果:经三年以上随访,内固定愈合56例,占84%;股骨头坏死20例,占29%;髋关节功能良好55例(包括股骨头置换病例)占71%.

结论:闭合复位内固定适用于大多数移位性股骨颈骨折或全身情况不能耐受人工股骨头置换术,或者不愿意作人工股骨头置换者,但对头下型严重移位者仍应首选人工股骨头置换。辨证施治应用中药,早期作除伤肢以外的其他肢体活动有利于减少和防治并发症的发生。

Treatment of Senlie Femoral Neck Fracture

Abstract:

Objective:To explore the treatment methods of senile femoral neck fractrue.

Methods:77 cases were treated with operation and Traditional Chinese Medicine.Operation methods include percutanecous multiple screw pins(36 cases),percutanecous mult iple triang ular pins(6 cases),the co mbinat ion of compression screw with screw pins(25 cases)and the hemiarthroplasty(10 cases).

Results:During 3 to 7 years follow up,54 cases got complete union of bone(84%);Femoral head necrosis occurred in 20 cases(29%);55 cases gained favorabel hip function(71%,include the patients whose femoral head were replace mented).

Conclusion:The method of closed reduction and internal fixation is suitable for most displaced femoral neck fr acture and the patients whose health condition can't tolerate the replacement operation.But to subcapital type fracture combined with serious displacement,the hemiarthroplasty should be done.Also,Traditional Chinese Medicine can be used to reduce complication.
