折断加压螺钉治疗股骨颈骨折

目的:研究折断螺钉治疗股骨颈骨折的机理,临床操作方法及术前后注意事项,提高临床技能,减少股骨颈骨折的并发症。

方法:选择新鲜股骨颈骨折病人72例,术前胫骨结节牵引,牵引状态下C形臂透视复位,行患侧大腿外侧小切口,显露股骨上段,然后在与股骨夹角20°～30°方向打入三枚折断螺钉。

结果:72例中68例愈合,愈合率为9444%.

结论:选择合适长度的折断螺钉,把握合适的进针方向是手术成功的关键,也是骨折良好愈合的基础。而早期手术,术后不负重,患侧不侧卧,不盘腿是防止股骨头缺血坏死的必备条件。

Treatment of Femoral Neck Fractures with compression broken screw

Abstract:

Objective:The study is to describe the compression broken screw for the treatment of femoral neck fractures,and to explore the mechanisms of this method.

Methods:72 cases were chosen for the operation.After reduction under the traction of tubercle of tibia,three broken screw were driven into the femur from the area 2 3cm below the greater tuberosity towards the center of the femoral head with an angle of 20 30 degree.

Results:Of the 72 cases,68 patients obtained complete union of bone.The rate of bony union is 94 44%.

Conclusion:This method is suitable for patients under 70 ages.The length of the screw and the inserting direction of the pine determine the success of the operation,which is the base of the excellent fusion of the bone.In order to prevent from femur head necrosis,it is necessary for the patients to be treated early and not to weight bearing.
