特殊类型腰椎间盘突出症的诊断和治疗

报告经手术证实的腰椎间盘突出症325例中，属极外侧型14例（4.3%）、硬膜内型4例（1.2%）、神经根鞘内型2例（0.6%）、游离移位型8例（2.5%）.结合文献复习，分别讨论其发病机理、临床特点、影像表现、诊断及治疗。作者认为对详细病史及临床表现的综合分析是避免漏、误诊的关键; 术中探查阴性时有针对地以细橡皮导尿管做相应部位的探查, 是避免漏治的必要措施。

Diagnosis and Treatment of Special Lumbar Intervertebral Disc Herniation

Abstract:28cases out of 325 cases of lumbar intervertebral disc herniation,confirmed in operation,were reported.Among them,14 cases(4.3%)were the extremely lateral type,4 cases (1.2%)the intra durdl type,2 cases(0.6%)the intra sheath type,and 8 cases the dissociated and displaced type.The pathogenesis,clinical features,image presentation,diagnosis and treatment of the disease were discussed with literatural review.According to the authors'opinion,the key to avoid missed diagnosis or misdiagnosis was to analyse the detailed medical history and clinical presentation of the patient integratively.If no herniation was found in operation,the necessary measurement should be made,such as to explore relevant position with a thin rubber urinary catheter,in order to prevent missed diagnosis.
