目的:研究Kaneda器械在胸腰段骨折中的应用。

方法:对21例胸腰段脊柱骨折并截瘫,施行前路减压,Kaneda器械内固定的治疗。

结果:平均随访6个月,按Frankel分级,除3例A级,2例B级截瘫无好转外,余均有2级以上的改善,术中胸膜破裂2例,无大血管及脊髓神经损伤。

结论:脊髓前方受压者前路手术能直视下去除压迫,截瘫恢复率高。Kaneda器械能立即机械性重建脊柱,患者可提早活动,植骨块不易移位,保证了脊柱的融合。

Application of Kaneda Device to Thoracolumbar Fracture
Abstract:

Aim To study the application of Kaneda device to thoracolumbar fracture.

Methods 21 cases of thoracolumbar fracture complicated with paralysis have been treated with anterior decompression and Kaneda device internal fixation.

Results Following - up 6 months in average, the satisfactory Results were obtained. According to Frankel’s classification, the improvement of paralysis were higher than 2 grades in all patients, except 3 cases of grade A patients and 2 cases of grade B. During operation, ruptured pleura was occured in 2 cases, but no injury in spinal cord, nerves and large blood vessels.

Conclusion Anterior approach operation for anterior compression of spinal cord may relieve the compression under direct vision, with high recovery rate of paralysis. Kaneda device can reconstruct the spine immediately. The patients can do exercise earlier. The grafted bones are not easy to displace. thus ensuring the fusion of the spine.
