三维打印技术联合定制接骨板治疗髋臼骨折的临床应用

目的: 探讨应用三维打印技术联合定制接骨板在治疗髋臼骨折中的应用价值和临床疗效。

方法: 2020年6月至2022年6月11例髋臼骨折患者采用三维打印技术进行术前规划,同时应用定制接骨板方法治疗,男8例,女3例,年龄25~66岁。骨折按Letournel-Judet分型:后柱后壁骨折4例,T型骨折2例,横行后壁骨折2例,双柱骨折2例,前柱伴后半横骨折1例。记录患者手术时间、术中出血量、术中透视次数,术后引流量、术后骨折愈合时间、髋关节功能评分并进行分析。

结果: 11例患者手术时间80~150 min,术中出血量150~700 ml,透视次数2~6次,术后引流量60~195 ml,骨折愈合时间2.5~6.0个月。按Matta评分评价骨折复位:解剖复位3例,满意复位8例。11例患者获得随访,时间7~18个月。术后6个月髋关节Merle d'Aubigne功能评分:优6例,良3例,可1例,差1例。1例出现切口脂肪液化,1例出现闭孔神经牵拉症状。

结论: 应用三维打印技术联合定制接骨板治疗髋臼骨折方法有效,另外,打印模型能够为术者提供骨折立体形态结果,结合术前模拟,便于手术复位,有效提高手术效率。

Clinical application of three-dimensional printing technology combined with customized bone plate in the treatment of acetabulum fracture
ABSTRACT  

Objective To explore the application value and clinical effect of 3D printing combined with customized bone plate in the treatment of acetabular fracture.

Methods From June 2020 to June 2022,11 patients with acetabular fractures underwent preoperative planning using 3D printing technology and were treated with customized bone plates including 8 males and 3 females,aged 25 to 66 years old. The fractures were classified according to Letournel-Judet:4 posterior wall fractures,2 T-type fractures,2 transverse posterior wall fractures,2 double column fractures,and 1 anterior column with posterior semi-transverse fractures. The operative time,intraoperative blood loss,intraoperative fluoroscopy times,postoperative drainage volume,postoperative fracture healing time,and hip function score were recorded and analyzed.

Results The operation time of 11 patients was 80 to 150 min,intraoperative blood volume was 150 to 700 ml,fluoroscopy frequency was 2 to 6,postoperative drainage flow was 60 to 195 ml,and the fracture healing time was 2.5 to 6.0 months. Fracture reduction was evaluated according to Matta score:anatomical reduction in 3 cases and satisfactory reduction in 8 cases. Eleven patients were followed up for 7 to 18 months. The hip Merle d'Aubigne function scores were excellent in 6 cases,good in 3 cases,fair in 1 case and poor in 1 case. Incision fat liquefaction occurred in 1 case and obturator nerve traction in 1 case.

Conclusion The application of 3D printing technology combined with customized bone plates in the treatment of acetabular fracture is effective. In addition,the printed model can provide the operator with the results of the three-dimensional shape of the fracture,which is convenient for surgical reduction and effectively improves the efficiency of surgery.
