掌长肌肌腱结合聚酯缝线重建喙锁韧带治疗RockwoodⅢ型肩锁关节脱位

目的:探讨掌长肌肌腱结合聚酯缝线重建喙锁韧带治疗肩锁关节脱位的临床疗效。

方法:自2011年8月至2013年11月, 采用掌长肌肌腱结合聚酯缝线重建喙锁韧带治疗RockwoodⅢ型肩锁关节脱位患者37例, 其中男24例, 女13例;年龄19~46岁, 平均32岁;左侧11例, 右侧26例。29例新鲜脱位, 8例陈旧性脱位。观察术中出血量、手术时间, 术后按照Karlsson 标准评定肩关节功能恢复情况。

结果:37例均获随访, 时间2.5~5个月, 平均3.5个月。手术时间52~98 min,平均72 min;术中出血量50~180 ml,平均75 ml;37例手术切口均Ⅰ期愈合。根据Karlsson疗效评定标准, 优32例, 良5例。

结论:对于RockwoodⅢ型肩锁关节脱位, 采用掌长肌肌腱结合聚酯缝线重建喙锁韧带, 具有手术简单, 术后肩锁关节功能恢复好的优点。

Reconstructing coracoclavicular ligament in treating Rockwood-Ⅲ Acromioclavicular dislocation by palmaris longus muscle with polyester suture
ABSTRACT  

Objective:To explore therapeutic effects of reconstructing coracoclavicular ligament for the treatment of Rockwood-Ⅲ Acromioclavicular dislocation by palmaris longus muscle with polyester suture.

Methods:From August 2011 to November 2013, 37 cases with Rockwood-Ⅲ acromioclavicular dislocation were treated with reconstructing coracoclavicular ligament by palmaris longus muscle with polyester suture. Among patients, 24 were males and 13 were females, ranging the age from 19 to 46 years old, with an average of 32 years old. There were 11 cases on the left side and 26 cases on the right side. Twenty-nine cases were fresh dislocation and 8 cases were old dislocation. Blood loss, operative time were observed, and Karlsson evaltae standard were applied for assessing postoperative recovery of shoulder joint function.

Results:All patients were followed up from 2.5 to 5 months with an average of 3.5 months. Operative time ranged from 52 to 98(meaned 72) min, blood loss ranged from 50 to 180(meaned 75) ml. All operative incision were healed at the satgeⅠ。 According to Karlsson standard, 32 cases obtained excellent results and 5 cases were moderate.

Conclusion:For Rockwood-Ⅲ acromioclavicular dislocation, reconstructing coracoclavicular ligament by palmaris longus muscle with polyester suture has advantages of simple operation, and rapid recovery of shoulder joint function.
