目的：回顾和总结胫骨PilonⅡ型及Ⅲ型骨折的手术治疗经验,预防各种并发症.
方法：分析自2006年3月至2011年10月收治的29例患者,男23例,女6例;年龄21～61岁,平均35岁.按Ruedi-Allgower分型:Ⅱ型19例,Ⅲ型10例.所有患者采用切开复位钢板内固定手术治疗,苜蓿叶形钢板内固定8例,解剖型钢板内固定10例,锁定解剖钢板固定11例.观察治疗效果及并发症情况.
结果：术后随访12～42个月,平均15个月,疗效评定参照Mazur评价标准,其中Ⅱ型优12例,良5例,可2例;Ⅲ型优4例,良4例,可1例,差1例.全部达到骨性愈合,时间12～32周,平均15周.
结论：严格把握手术时机和手术适应证,规范手术操作,掌握手术技巧,可有效避免并发症,取得比较满意的治疗效果. |

| Treatment of Pilon fractures of typeⅡ and Ⅲ with operation |

ABSTRACT
Objective：To review and conclude the experience and to prevent a variety of complications of operative treatment for tibial Pilon fractures of typeⅡ and Ⅲ.
Methods：From March 2006 to October 2011,conse cutive 29 patients were treated in our center.According to Ruedi-Allgower classification,there were 19 patients of typeⅡ and 10 patients of typeⅢ.There were 23 males and 6 females,ranging in age from 21 to 61 years old.The average age was 35 years old.All the patients were treated by open reduction and internal fixation(ORIF).Cloverleaf plate was applied in 8 cases,anatomical plate in 10 cases,and anatomical locking plate in 11 cases.Results of treatment and complications were thoroughly documented and analyzed.
Results：All the patients were followed up,and the duration ranged from 12 to 42 months,averaging 15 months.Referring to Mazur criterion,12 patients gained excellent result,5 good and 2 fair in typeⅡ fractures;and 4 excellent,4 good,1 fair and 1 poor in typeⅢ fractures.All factures healed during 12 to 32 weeks,and the average time was 15 weeks.
Conclusion：Good therapeutic effects can be obtained by grasping the timing of surgery and indications strictly,standardising the operative manipulation and gripping the tip of operation. |
